By accessing and using the services of Vibrant Health Guides, you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by these terms, please do not use our services.
2. Medical Services Disclaimer
The information provided by Vibrant Health Guides is for general informational and educational purposes only. All information is provided in good faith, however, we make no representation or warranty of any kind regarding the completeness, accuracy, reliability, suitability, or availability of the information. The information contained on this website is not a substitute for professional medical advice, diagnosis, or treatment.
3. Patient Responsibilities
Patients are responsible for providing accurate and complete medical history, arriving on time for appointments, providing 24-hour notice for cancellations, following prescribed treatment plans, and making timely payments for services rendered.
4. Appointment Policies
Appointments must be scheduled in advance. Same-day appointments are subject to availability. Patients who are more than 15 minutes late may need to reschedule. No-shows or late cancellations (less than 24 hours notice) may incur a $50 fee.
5. Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept major insurance plans, credit cards, debit cards, and cash. Patients are responsible for all co-pays, deductibles, and non-covered services. Unpaid balances may be sent to collections.
6. Prescription Refills
Prescription refill requests require 48-72 hours for processing. Controlled substances cannot be refilled via phone or portal and require an office visit. We reserve the right to refuse prescription refills if clinical evaluation is needed.
7. Telehealth Services
Telehealth services are provided via secure, HIPAA-compliant video platforms. Patients must be physically located in Nebraska during telehealth visits. Not all conditions can be treated via telehealth, and in-person visits may be required.
8. Limitation of Liability
Vibrant Health Guides shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use or inability to use our services, unauthorized access to your data, or any other matter relating to our services.
9. Privacy and Confidentiality
We are committed to protecting your privacy and maintaining the confidentiality of your medical information in accordance with HIPAA regulations and our Privacy Policy. By using our services, you consent to our collection and use of information as described in our Privacy Policy.
10. Modifications to Terms
We reserve the right to modify or replace these Terms at any time at our sole discretion. If a revision is material, we will provide at least 30 days' notice prior to any new terms taking effect. By continuing to use our services after those revisions become effective, you agree to be bound by the revised terms.
11. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Nebraska, without regard to its conflict of law provisions. Any legal actions or proceedings arising out of these Terms shall be brought exclusively in the courts located in Knox County, Nebraska.
